Ortho Warranty:
Billing & Payment Policy – GSR Dental LLC
1. Billing & Payment Due Date
All accounts are due and payable upon receipt of each monthly invoice or statement.
Billing begins on the invoice issue date.
2. Payment Terms
Standard terms: Net 25 (payment due within 25 days from the start of the invoice date).
New clients: Net 15 (payment due within 15 days from invoice date).
After two timely and successful payments, clients will be extended Net 25 terms.
3. Late Fees & Penalties
Update: August, 2025.
A minimum service charge of 1.5% per month (18% per annum) or the maximum rate permitted by Texas Law, whichever is less, will be applied to all balances more than 30 days past due. If any provision of this rate exceeds the lawful maximum, the charge will automatically be reduced to the highest rate allowed by law.
Credit Card Payment On Late Fee: If you are late on payment, there may be a credit card payment fee of (3%) for processing cost. This fee is not assessed on debit cards or cash payments and will be clearly disclosed before the transaction is completed.
Accounts not paid within stated terms may be placed on C.O.D. (Cash on Delivery) status, after 60 days.
4. Ownership & Returned Payments
All materials and items remain the property of GSR Dental LLC until the client’s account is paid in full.
Returned checks will incur a minimum charge of $30.00.
5. Collections & Legal Fees
Clients agree to pay all applicable attorney’s fees, court costs, collection fees, and any other expenses incurred in recovering past due balances or insufficient fund checks, as permitted by law.

No-Fault Remake Policy
GSR Dental LLC offers remakes or adjustments at no additional charge within the warranty period if the original appliance is returned.
Chairside adjustments and deliver appliance to patient will incur additional charges.
Limited Warranty & Liability
GSR Dental LLC warrants that all devices are made according to your specifications and approval for intended use.
No other warranties are provided, including merchantability or fitness for a particular purpose.
Devices failing under normal wear and tear may be repaired or replaced at no charge for material and workmanship, up to three months for Hawley retainers.
Refund Policy
Refunds are available within 25 days from the invoice date.
The original appliance must be returned for a refund to be processed.
Exclusions & Special Conditions
No warranty or refund applies if the appliance was chewed or eaten by an animal or returned catastrophically damaged (multiple broken pieces).
Patients should be advised to keep appliances away from pets, as they are commonly attracted to them.
